• They followed back up a few weeks later and gave the same rating. 
• According to documents, staff was not taking steps to "prevent illness by having repeated risk factors and good retail practice violations."
• They got dinged on things like food being stored wrong, cleaning solution not being mixed well, and food not having proper labels.
